The pick block is welded onto the drum or the cutting head, and the sleeve inside it is what the pick shank actually turns in: when a pick stops rotating and wears to a point on one side, the sleeve is usually the part that has gone oval, not the pick. Blocks and sleeves come in the machine's own patterns — U80, U90, U170, K177, K225 — and the pattern has to match both the pick and the drum, so a block bought on price and welded into the wrong position costs a set of picks and a rework of the head.
